About the Practice

Julie Hjelm Physiotherapy is a mobile rehabilitation clinic serving seniors and people with mobility challenges across Victoria and surrounding areas. We provide in-home physiotherapy to individuals with complex health needs, supporting mobility, independence, and quality of life across private homes, independent living, long-term care, and memory care settings.

Our approach is functional and fitness-forward, grounded in helping patients maintain meaningful independence within the realities of aging, comorbidities, and changing health status.

The Role

We are seeking an experienced physiotherapist to deliver care in a fully mobile, community-based setting.

This role involves working independently in patients’ homes, often with medically complex seniors, without on-site physiotherapy support. It is best suited to clinicians who are confident managing uncertainty, making autonomous clinical decisions, and adapting care in dynamic environments.

Clinical Population

You will commonly work with:

  • Frail seniors with multiple comorbidities (cardiometabolic, neurological, MSK overlap)
  • Post-hospital discharge and deconditioning
  • Falls risk, mobility decline, and balance impairments
  • Cognitive impairment (e.g., dementia) impacting treatment planning
  • Clients with environmental and social barriers to care

Key Responsibilities

  • Independently assess, diagnose, and manage patients in home and residential care settings
  • Develop and adapt individualized treatment plans based on complex and evolving presentations
  • Prescribe and progress functional exercise programs tailored to real-world environments
  • Identify clinical red flags and determine when escalation or referral is required
  • Communicate effectively with families, caregivers, and interdisciplinary providers
  • Coordinate care with kinesiologists and other team members as appropriate
  • Maintain clear, timely, and defensible clinical documentation

Qualifications

Required:

  • Registered Physiotherapist in good standing with CHCPBC
  • Minimum 2+ years of relevant clinical experience, ideally including:
  • Community or home health physiotherapy
  • Hospital (acute care, rehab, or discharge planning)
  • Geriatrics and/or neurological populations
  • Demonstrated ability to manage complex patients independently
  • Strong clinical reasoning, time management, and risk assessment skills
  • Valid driver’s license and access to a vehicle
    Please note: This role is not suitable for clinicians requiring close on-site supervision.
